EBMT 2026 | Is there still a curative role for allogeneic transplantation in multiple myeloma?

In this video, Daniele Avenoso, MD, University of Brescia, Brescia, Italy, discusses whether there is still a curative role for allogeneic stem cell transplantation (alloSCT) in multiple myeloma (MM) in the era of novel therapies. Dr Avenoso highlights that while alloSCT is becoming less frequently used, it may still be a valuable option for patients who relapse following CAR T-cell therapy or bispecific antibodies. This interview took place at the 52nd Annual Meeting of the EBMT in Madrid, Spain.

Well, this is a matter of debate also because the treatment landscape of multiple myeloma is changing thanks to the availability of newer drugs, bispecific antibodies and CAR-T therapies. So we wanted to address the questions of how do patients perform that receive an allo transplant in an era where CAR-T therapies and new drugs were not available. And in the context of matched sibling donor, even in the presence of a very good partial response, the allogenic hematopoietic stem cell transplant can cure myeloma. It can cure myeloma because there is an advantage of survival and even the progression-free survival shows a very interesting trend. We need to be honest that the transplant cohort belongs to the past. Nowadays, transplantation is more rare due to the availability of safer treatments, but I think maybe in the near future we will discuss again allografting in myeloma patients, maybe at relapse after CAR-T therapies and bispecific antibodies.
