EBMT 2026 | Results from the Phase II EMN19 study: Dara-VCd in patients with extramedullary myeloma

So, EMN19 is the first prospective trial just focusing on extramedullary multiple myeloma. When we started this trial, under those pieces of the European Myeloma Network before the era of the COVID pandemic, we were not aware of the prognostic value of paraosseus versus real soft tissue extramedullary disease. But nevertheless, two-thirds of the patients included in this trial have soft tissue plasmacytomas...

So, EMN19 is the first prospective trial just focusing on extramedullary multiple myeloma. When we started this trial, under those pieces of the European Myeloma Network before the era of the COVID pandemic, we were not aware of the prognostic value of paraosseus versus real soft tissue extramedullary disease. But nevertheless, two-thirds of the patients included in this trial have soft tissue plasmacytomas. And among 40 patients, which are recruited from three countries, from Turkey, Greece, and Italy, we were able to show that continuous treatment with daratumumab in combination with VCD, which is given once a week. But please bear in mind, this treatment is given continuously for three years. When we started this trial, continuous treatment for three years with such an approach was not really very well established. And with this type of treatment, we were able to demonstrate and achieve MRD negativity in the marrow and plus a complete metabolic response when combined together achieves 100% progression-free survival. And on top of this, we were also able to demonstrate that the number of plasmacytomas has an impact, which is a kind of a reflection of the tumor burden. And also, for the first time, just focusing explicitly on extramedullary disease, we measured circulating tumor cells. And unexpectedly, we were able to see no difference between a historical control group in the number of circulating tumor cells in the blood among extramedullary multiple myeloma presenting myeloma patients. So this may be discussed within the context of only 40 patients, but it may be pioneering work for future studies.
