Current challenges and future strategies for TP53-mutated AML and MDS

Maximilian Stahl, MD, Yale Cancer Center, New Haven, CT, presents on the current challenges and future strategies in the treatment of patients with TP53-mutated acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS). Dr Stahl explains that allogeneic stem cell transplant (alloSCT) remains the central treatment for eligible patients, including those with biallelic TP53 mutations. He discusses the use of hypomethylating agents plus venetoclax as a bridge to alloSCT, hypomethylating agent monotherapy for transplant-ineligible patients, and recommends avoiding chemotherapy in most cases. He emphasizes that clinical trials are the preferred option for these patients, ideally with event-based endpoints such as overall survival. This presentation took place at the 8th International Workshop on Acute Leukemias (iwAL) in Cambridge, MA.
