ASCO 2026 | Updated results from the inMMyCAR trial: in vivo CAR-T therapy KLN-1010 in R/R multiple myeloma

P. Joy Ho, MBBS, DPhil, FRACP, FRCPA, FFSc(RCPA), Royal Prince Alfred Hospital & University of Sydney, Sydney, Australia, presents the updated results from the inMMyCAR trial (NCT07075185), evaluating the in vivo CAR-T therapy KLN-1010 in patients with relapsed/refractory (R/R) multiple myeloma (MM). Prof. Ho highlights that all evaluable patients have achieved early measurable residual disease (MRD)-negative responses and, encouragingly, those with the longest follow-up remain MRD-negative. She also notes that KLN-1010 is generally well-tolerated, with manageable toxicities. This interview took place during the 2026 American Society of Clinical Oncology (ASCO) Meeting in Chicago, IL.

The inMMyCAR study looks at an in-vivo CAR-T, which is KLN-1010. And at ASCO this year, we have updated those results and now include a total of 18 patients where we had previously presented four patients at ASH. And this shows very promising activity in the sense that we continue to show 100% MRD negative marrow responses in these patients who are three or more prior lines of therapy and 56% triple class refractory. So with this in mind, we now have much longer follow-ups. So the first four patients are all MRD negative to the level of 10 to the minus six at six months, while the first two patients are continuing in stringent complete response beyond nine months. We’ve also shown very good safety profiles with cytokine release syndrome only mild and there are no severe cases. We have now got two cases of ICANS, but both were highly manageable, including one grade 3 ICANS, which completely resolved, and one grade 1. And overall, we also showed really amazing CAR-T expansion and in particular persistence, which I think bodes well for the future in terms of continuing and durable remissions.
