In this video, Lukas John, MD, University Hospital Heidelberg, Heidelberg, Germany, discusses genetic heterogeneity, which is often seen in myeloma, by focusing on a study that investigated mutations in focal lesions using various gene sequencing methods. First, Dr John mentions the importance of studying these focal lesions, their heterogeneity, and the potential differences in the immune microenvironment of these cells. Dr John further explains the methodology used in this study, as well as the results, including confirmation of the presence of genetic heterogeneity and the various processes that may be involved. Dr John then discusses the differences seen in the immune microenvironment of these focal lesions, including depletion of macrophages and distribution of clonal T cells. To conclude, Dr John presents some data from a patient in this study with multiple relapses, the role that focal lesions played in these relapses, and how further research into focal lesions may contribute to the future of myeloma treatment.
