Patrizia Chiusolo, MD, PhD, Agostino Gemelli University Policlinic, Rome, Italy, discusses findings from the EBMT Chronic Malignancies Working Party study comparing post-transplant cyclophosphamide (PTCy) to other graft-versus-host disease (GvHD) prophylaxis strategies in patients with myelofibrosis (MF) undergoing matched-related donor allogeneic stem cell transplantation (alloSCT). This interview took place at the 52nd Annual Meeting of the EBMT in Madrid, Spain.

In matched related donor, the problem is that PTCy has been used in, for example, other malignancies and is well defined the role that PTCy has in this setting. In myelofibrosis, instead, the results are limited by small casistics. And for this reason, we presented this data from the registry EBMT on 902 patients, of which 58 were treated with PTCy. Again, in this study, we demonstrated that there is a significant reduction of chronic Graft-versus-Host disease in patients treated with PTCy, but also of acute Graft-versus-Host disease. Regarding instead the incidence of relapse, we registered a higher rate as compared to ATG but also to conventional prophylaxis added with calcineurin inhibitor with methotrexate or MMF.
