EBMT 2026 | Comparing PTCy versus ATG as GvHD prophylaxis in sibling and MUD transplantation for myelofibrosis

Patrizia Chiusolo, MD, PhD, Agostino Gemelli University Policlinic, Rome, Italy, briefly discusses the findings of a study comparing post-transplant cyclophosphamide (PTCy) and anti-thymocyte globulin (ATG) as graft-versus-host disease (GvHD) prophylaxis in sibling and matched unrelated donor (MUD) stem cell transplantation for myelofibrosis. This interview took place at the 52nd Annual Meeting of the EBMT in Madrid, Spain.

Yesterday, I presented data from the International Consortium of Myelofibrosis, which collects data from different centers in Europe and the United States. In this paper and abstract we presented, we evaluated the role of PTCy and ATG as a platform of prophylaxis of graft-versus-host disease in patients with a matched donor, both related and unrelated. What was important in the study was that we registered a higher rate of reduction of chronic graft-versus-host disease in patients treated with PTCy, while we had a trend to an increased rate instead of relapse in the same setting of patients...

What was important in the study was that we registered a higher rate of reduction of chronic graft-versus-host disease in patients treated with PTCy, while we had a trend to an increased rate instead of relapse in the same setting of patients. With ATG instead, the role was not well defined, but also we had a reduced incidence of relapse. In terms of graft-relapse-free survival, we did not show any difference between the two kinds of GVHD prophylaxis.
