SOHO 2026 | Evaluating pirtobrutinib for patients with R/R MZL exposed to BTKis: BRUIN cohort data

Krish Patel, MD, Sarah Cannon Research Institute, Nashville, TN, discusses cohort data from the BRUIN trial (NCT03740529), evaluating pirtobrutinib activity in relapsed/refractory marginal zone lymphoma (MZL). Dr Patel highlights that the strength of the BRUIN data lies in its focus on MZL, making it the strongest data available for patients with prior exposure to covalent BTK inhibitor (BTKi) therapy. This interview took place at the 14th Annual Meeting of the Society of Hematologic Oncology (SOHO 2026) in Houston, TX.

Yeah, so marginal zone lymphoma is often a disease where most of our therapies are inferred from trials in follicular lymphoma. Yeah, so marginal zone lymphoma is often a disease where most of our therapies are inferred from trials in follicular lymphoma. So to really have data specific to that population of patients and in a specific scenario after prior covalent BTK inhibitor, I think, really is unique. And so for me, personally, I think that the strength of that data is that it is really focused on marginal zone lymphoma. And so my own sequencing approach really is after a covalent BTK inhibitor, which I use quite commonly in marginal zone lymphoma, I think the data for pirtobrutinib is the strongest we have. And certainly we can see now we have CAR T-cells available as well in marginal zone lymphoma. But the population of patients enrolled in those trials is a little bit different. So I think for a patient who’s already had covalent BTK inhibitor, the strongest data we have really comes from that BRUIN experience.
