IMS 2026 | Tailoring treatment to all patient profiles in transplant-ineligible newly diagnosed myeloma

Claudio Cerchione, MD, PhD, Istituto Scientifico Romagnolo per lo Studio e la Cura dei Tumori (IRST) IRCCS, Meldola, Italy, discusses how clinicians should tailor treatment to address all patient profiles in transplant-ineligible newly diagnosed multiple myeloma. Dr Cerchione highlights key regimens currently available, explains the emerging concept of a transplant-deferred population, and notes the potential of on-body injection (OBI) to optimize treatment delivery, tolerability, and quality of life. This interview took place at the 23rd International Myeloma Society (IMS) Annual Meeting in Glasgow, UK.

In transplanting eligible newly diagnosed patients, we have currently available a double quadruplet, IsaVRd and daraVRd, respectively from IMROZ and CEPHEUS studies. A new concept is emerging, the idea of transplant-deferred population, and this is another important point because the grade zone can start the quadruplet and then decide if transplanting or not. The idea is that also in this population the mission is changing and the endpoint is not only controlling the disease, blocking the disease, but the idea that the mission is to obtain the deepest response as possible, MRD negativity...

The idea is that also in this population the mission is changing and the endpoint is not only controlling the disease, blocking the disease, but the idea that the mission is to obtain the deepest response as possible, MRD negativity. In this symposium today we have spoken about the idea of driving the patient to OBI to optimize the treatment delivery and in this way the tolerability, the quality of life, and we hope in the next future also to drive this treatment in a domestic setting because we know that there are in IZALCO and IRAKLIA trials also some patients treated at home thanks to OBI. I think that this tool, on-body injection, can help also some patients that are afraid of needles and in some way can optimize the space and the time of healthcare professionals and also the satisfaction of both physicians, nurses, and patients. And I think that this can be a potential game changer for all isatuximab-based treatments, thanks to the safety profile, thanks to the simple way of use, and I think that this can optimize, also in the next future, the idea of cost-effectiveness, the use of drugs, not only in front-line settings, but where isatuximab is used in the panorama of multiple myeloma.
