IMS 2026 | TRIlogy-1 trial: updated ramantamig safety and efficacy data in relapsed/refractory multiple myeloma

Jeffrey Matous, MD, discusses updated safety and efficacy data for ramantamig, a dual-targeting BCMA- and GPRC5D-directed T-cell-redirecting antibody, from the Phase I TRIlogy-1 trial (NCT05652335) in patients with relapsed/refractory (R/R) multiple myeloma. He highlights the absence of new safety signals and suggests that treatment-related adverse effects, including taste disturbances associated with GPRC5D targeting, may be less frequent than with earlier agents targeting these antigens. This interview took place at the 23rd International Myeloma Society (IMS) Annual Meeting in Glasgow, UK.

Yeah, at the IMS conference, we were fortunate enough to present updated safety and efficacy data with Romantamig in the TRIlogy-1 trial. This is a phase one study looking at the first-in-class dual targeting BCMA and GPRC5D T-cell redirecting antibody ramantamig. And in this study, we had an additional 20 patients reported beyond what has been reported at prior meetings. And the focus of this additional patient recruitment at the recommended phase two dosing was to develop an outpatient program or outpatient feasibility assessment, if you will, for ramantamig. And so with regards to safety, no new safety signals were seen. And of course, we see the usual BCMA and GPRC5D targeting side effects that were present. But it’s my bias, and I think the data bear this out, that both of these side effects are less than what we see when we look at the data for the first generation, for example, teclistamab, belantamab, or talquetamab. And in particular, the on-tumor, on-target, off-tumor side effects of targeted GPRC5D, such as taste disorder, are less with ramantamig than with talquetamab.
