iwAL 2026 | Sequencing targeted therapies in AML with multiple targetable alterations

Amir Fathi, MD, MPH, Massachusetts General Hospital, Boston, MA, discusses uncertainties around sequencing targeted therapies in acute myeloid leukemia (AML). He highlights the lack of definitive evidence to guide treatment for patients with multiple targetable alterations and discusses emerging trials evaluating combinations of menin and FLT3 inhibitors. I think the uncertainty regarding sequencing continues. We don’t yet have data to support whether somebody who has both, let’s say, an IDH mutation and a FLT3 mutation or has an NPM1 mutation and an IDH mutation, whether in the relapsed/refractory setting they should receive one therapy or another, or a combination. What I can say is that there are currently clinical trials looking at combinatorial therapies, including those with menin inhibitors and FLT3 inhibitors, such as quizartinib and gilteritinib...

What I can say is that there are currently clinical trials looking at combinatorial therapies, including those with menin inhibitors and FLT3 inhibitors, such as quizartinib and gilteritinib. I am hopeful that those trials will shed light on what the approach may be for these patients who have multiple targetable alterations.
