ERIC CLL 2026 | Frontline management of Richter’s transformation: current approaches and remaining uncertainties

Philip Thompson, MD, University of Melbourne and Peter MacCallum Cancer Centre, Melbourne, Australia, discusses the current state of frontline management of Richter’s transformation, noting that the field agrees the standard of care is inadequate and outcomes with chemoimmunotherapy are poor due to the disease’s intrinsic chemoresistance. Dr Thompson highlights that clinical trials have shown promising results with additions to chemoimmunotherapy, and combinations of small molecules with checkpoint inhibitors, as well as the emerging use of bispecific antibodies. This interview took place at the 2026 ERIC International Meeting in Barcelona, Spain.
