Thomas Kipps, MD, PhD, UC San Diego, Moores Cancer Center, La Jolla, CA, comments on the use of venetoclax (ven)-based regimens for the treatment of patients with relapsed/refractory (R/R) chronic lymphocytic leukemia (CLL). In a single-center retrospective analysis, efficacy data were assessed from patients treated with ven-based regimens of either ven monotherapy or ven as a combination therapy with rituximab, obinutuzumab, or ibrutinib. High undetectable measurable residual disease (MRD) rates and deep remissions were observed, with combination regimens demonstrating superior efficacy to ven monotherapy. The presence of MRD in patients at 12 months after cessation of ven treatment was a prognostic factor for progression-free survival (PFS), highlighting the importance of its use in monitoring responses to treatment. However, Dr Kipps acknowledges that implementing an MRD-guided strategy into real-world clinical practice may be difficult, and a compromise must be made.
