IMS 2025 | Combined use of 18F-FDG-PET/CT, whole-body diffusion-weighted MRI and MRD by NGS in NDMM

Marco Talarico, MD, University of Bologna, Bologna, Italy, discusses the combined use of 18F-FDG-PET/CT, whole-body diffusion-weighted MRI and measurable residual disease (MRD) by next-generation sequencing (NGS) in newly diagnosed multiple myeloma (NDMM). Dr Talarico highlights a high concordance rate between the two imaging techniques and notes that there is a complementarity between functional imaging and bone marrow MRD evaluation. This interview took place at the 22nd International Myeloma Society (IMS) Annual Meeting in Toronto, Canada.

In Bologna we are conducting a prospective monocenter study aimed at comparing the diagnostic performance of PET-CT and whole body diffusion weighted MRI both at staging and in response assessment. So up to September 2025 we have 71 patients who have reached the re-evaluation phase and among these patients 45 have repeated both the imaging techniques since they were positive at baseline and we have observed a concordance of 93% translating into a [inaudible] which is a statistic index regarding concordance of 0...

In Bologna we are conducting a prospective monocenter study aimed at comparing the diagnostic performance of PET-CT and whole body diffusion weighted MRI both at staging and in response assessment. So up to September 2025 we have 71 patients who have reached the re-evaluation phase and among these patients 45 have repeated both the imaging techniques since they were positive at baseline and we have observed a concordance of 93% translating into a [inaudible] which is a statistic index regarding concordance of 0.7 which is a very good concordance between these two imaging techniques. Among patients achieving a response by both PET-CT and the whole body diffusion-weighted MRI and achieving at least a very good partial response by a hematologic standpoint, the assessment of minimal residual disease by next-generation sequencing was negative in about 75% of patients and positive in about one quarter of patients. So there is a very good concordance between the two functional imaging techniques and there is a complementarity between functional imaging, both FDG PET-CT and MRI, and bone marrow MRD evaluation. So one will not exclude the other.
