EBMT 2026 | Insights into the HCT-FS: implementing a frailty program in a transplant unit

María Queralt Salas, MD, Hospital Clínic de Barcelona, Barcelona, Spain, briefly discusses the implementation of the hematopoietic cell transplant frailty scale (HCT-FS) for patients undergoing stem cell transplantation at her institution, as frailty has previously been shown to negatively impact survival outcomes. Dr Salas highlights favorable outcomes amongst patients in response to the program, including improved fitness status and fewer transplant-related toxicities. This interview took place at the 52nd Annual Meeting of the EBMT in Madrid, Spain.

Well, this has been a work done for our team during the last five years. We started implementing the hematopoietic cell transplant frailty scale at first consultation and transplant admission and at a few points after the procedure, day plus one hundred, six months and one year. So after assessing frailty for one year, we could see that this syndrome negatively affects the probability of survival. So we decided to implement an intervention with physical activity to try to train patients for the patients and help them to cope with the procedure. So patients are seen by the rehabilitation unit, and we prescribe physical activity using an app, sending videos, training videos to patients, they do it at home with their caregivers, and then we assess frailty again with the scale to check the effectiveness of the program. So we are seeing that patients are more committed to the transplant, they are in a better fitness status and the toxicities that we document after the procedure are less if you implement this rehabilitation.
