Martin Griesshammer, MD, Johannes Wesling Medical Center, Minden, Germany, highlights the unanswered questions surrounding the management of pregnant patients with myeloproliferative neoplasms (MPNs), particularly regarding the use of interferon as cytoreductive therapy. Prof. Griesshammer mentions a European consensus project that is reviewing available literature to address questions around interferon treatment in pregnancy and during breastfeeding, anticoagulation therapy, and other concerns. This interview took place at the European School of Haematology (ESH) 4th How to Diagnose and Treat: CML/MPN meeting in Vienna, Austria.

There are some unmet needs, especially the question of when and how to use cytoreductive drugs, especially the only drug you can use is interferon, obviously. And so we have to define in which patients we have to use interferon and when to use it. So normally we want you to start interferon half a year beforehand because if you start during pregnancy, the effect is very low because interferon takes a lot of time to work. Another important question is, is it possible for these affected women to breastfeed while taking interferon? So there are pros and cons and a lot of discussion around this topic and we don’t have a clear answer. So we are working on this within a consensus group, a European consensus group. But this is an open question. And there are many others, when to use aspirin, when to use heparin. So, we have at the moment a European consensus project with many experts together with some Canadian experts where we try to answer these questions on the basis of the literature.
