CAR-T Meeting 2026 | The potential of dual-targeting CAR-T approaches in R/R LBCL

Gloria Iacoboni, MD, PhD, Vall d’Hebron Institute of Oncology, Barcelona, Spain, discusses the potential of dual-targeting CAR T-cell therapy approaches in relapsed/refractory (R/R) large B-cell lymphoma (LBCL), highlighting the emergence of various CD19/CD20 targeted CAR-T constructs, including tandem and bicistronic designs. Dr Iacoboni expresses her excitement about future randomized clinical trials comparing the efficacy of these dual-targeted products with that of conventional CD19-directed CAR-T. This interview took place at the EBMT-EHA 8th European CAR T-cell Meeting, held in Palma de Mallorca, Spain.

Regarding this dual-targeting approach that zamto-cel presented, so as mentioned, this is a second generation CAR T-cell construct with a 4-1BB co-stimulatory domain targeted, you know, directed towards CD19/CD20 tandem construct. Regarding this dual-targeting approach that zamto-cel presented, so as mentioned, this is a second generation CAR T-cell construct with a 4-1BB co-stimulatory domain targeted, you know, directed towards CD19/CD20 tandem construct. It’s very interesting to see that there’s now a wealth of different CD19/CD20 targeted constructs, many presented at ASH and prior meetings in 2025. So different companies in different ways: bicistronic, tandem, usually, you know, CD19/CD20 directed are presenting, you know, efficacy and toxicity data in single arm clinical trials with these constructs. And I think, you know, it’s going to be a very exciting pathway forward because in the following years, we are going to see randomized clinical trials, head-to-head CAR-T versus CAR-T trials, comparing these dual-targeted CD19/CD20 products with our, let’s call them now conventional CD19-directed, available, commercially available CAR-T constructs, you know, axi-cel cell and liso-cel in the relapse refractory large B-cell lymphoma settings. So looking forward to the results to come.
