EBMT 2025 | The long-term outcomes and QoL with treosulfan-based conditioning in hematological malignancies

In this video, Filippo Milano, MD, PhD, Fred Hutchinson Cancer Research Center, Seattle, WA, briefly comments on the findings of a retrospective analysis that assessed the long-term outcomes and quality of life (QoL) with treosulfan-based conditioning in patients with hematological malignancies. Dr Milano highlights a surprising finding that 16 of the patients who underwent transplantation and were included in the analysis were able to have spontaneous pregnancies, a rare occurrence given the adverse effect on gonadic function associated with conditioning regimens. This interview took place at the 51st Annual Meeting of the EBMT in Florence, Italy.

We were lucky enough in Seattle to have treosulfan available for many years. The drug has been recently approved by the FDA, so now it’s available in other centers. And I recently combined the results of over 300 patients undergoing stem cell transplant with different stem cell sources like matched-unrelated, matched-related and cord blood transplant using treosulfan as a base of the conditioning regimen.

The main findings of the study were that quality of life was similar to the use of other conditioning regimens, but definitely the main finding that was extremely surprising, we observed that in 16 patients who underwent transplants in a period between 2006 and 2020, there were 16 patients that were able to have spontaneous pregnancies. And that’s quite amazing considering usually the use of conditioning regimens kind of ablates the gonadic function so and not allowing our patients to have pregnancies later on. So that’s definitely the most important finding we were able and very happy to see that.
