ESH AL 2026 | Significant advances made in Ph+ ALL: TKIs, immunotherapy & more

Josep-Maria Ribera, MD, PhD, Germans Trias i Pujol University Hospital, Barcelona, Spain, shares insights into the most significant advances made in the treatment of Ph-positive acute lymphoblastic leukemia (Ph+ ALL), highlighting the incorporation of TKIs into chemotherapy schedules, the use of third-generation TKIs, and the addition of immunotherapy. These approaches have led to reduced reliance on chemotherapy and allogeneic stem cell transplantation. This interview took place at the 5th How to Diagnose and Treat: Acute Leukemias meeting of the European School of Hematology (ESH AL) in Mandelieu-La Napoule, France.

So, my contribution to this meeting has been focused on Ph-positive ALL and is one of the subsets of ALL that has shown impressive advances in the recent years. First, the incorporation of TKI to the chemotherapy schedules. Second, the use of third-generation TKI, so modern TKIs with a wide spectrum and activity against resistance mutations. And more recently, the incorporation of immunotherapy together with TKI. And this has allowed to reduce to a minimum expression the chemotherapy. So this is a great advance for efficacy, for tolerability, and for lower use of allogeneic stem cell transplantation. That is an effective therapy, but also toxic. So the most important advance is now the combination of frontline chemotherapy, minimal chemotherapy, immunotherapy, and TKI, especially third-generation TKI. The results achieved are about 80% of overall survival, and really this is an unprecedented result now. And now, Ph-positive ALL is the most favorable subtype of acute myeloid leukemia now. And now, Ph-positive ALL is the most favorable subtype of acute myeloid leukemia in adults.
